Introduction

At Willpower Sports Rehabilitation & Conditioning, privacy of both clients and website visitors is important. When personal information is obtained, it is kept highly confidential and is accessible only by the client and William Dorman T/As Willpower Sports Rehabilitation & Conditioning. William Dorman is the data controller and is responsible for storing and processing your data in line with the data protection law. Who are we?

Willpower Sports Rehabilitation & Conditioning is a small business run by William Dorman as a sole trader.

What information do we collect?

Client information is collected in the following ways:

1.‘Send Us A Message’ website form.

When a prospective client completes the ‘Send Us A Message’ enquiry form on the website, the following information is collected and sent to William via email:

  • First and Last names

  • Contact telephone number

  • Email address

  • Any further information that the prospective client chooses to share via the ‘message’ text box.

This information is stored on William’s private email address. No other persons outside of the business have access to this email account.

2. 'Testimonials' form.

When a client completes the 'Testimonials' form on the website, the following information is collected and stored in a database to be able to subsequently display submitted testimonials on the website:

  • Name

  • Email address

  • Testimonial text.

The client's email address is not displayed on the website; it is collected so that the client's testimonial can be sent to William's private email address so that he can email back a response. No other persons outside of the business have access to this email account.

The client has an option to tick a check box within the 'Testimonials' form to allow their testimonial to be published on the website, or if preferred, to keep a testimonial private and be sent to William by email only.

3. Client Consent & Consultation Form.

At the start of every initial treatment, the client will be required to complete a Client Consent & Consultation Form, with further diagnostic/treatment information added by William during or following a treatment session. The client will be expected to provide the following information (as defined in the Clinical Record Keeping guidance issued by BASRaT [British Association of Sports Rehabilitators and Trainers], the UK regulator and professional association for Sports Rehabilitators):

  • Full name

  • Address

  • Date of birth

  • Gender

  • Contact telephone number

  • Email address

  • GP Name and Practice

  • Contraindications to the treatment

  • Current medication

  • Recent injury history

Any additional treatment visit will require an Additional Consultation Form to be completed, on which a reduced set of client data, containing further diagnostic/treatment information, will be recorded by William during or following the treatment session. Client Consent & Consultation Forms and Additional Consultation Forms are stored in a locked cabinet, which is only accessible to William.

Information is never collected from or shared with third parties, unless consent to do so with associated medical professionals has been provided by the client in advance by completing the relevant section on the Client Consent & Consultation Form.

How do we use personal information?

Contact details are collected to allow William to communicate with the client regarding the treatment. Medical information is collected to ensure that the treatment is safe, effective and appropriate.

How long do we keep your personal data for?

Timescales for retention of Client Consent & Consultation Forms are defined in the Clinical Record Keeping guidance issued by BASRaT (British Association of Sports Rehabilitators and Trainers), the UK regulator and professional association for Sports Rehabilitators:

For adults, Client Consent & Consultation Forms are stored for 8 years since the last dated record.

 

For children (U18), Client Consent & Consultation Forms are retained until the client’s 25th birthday, or 26th birthday if the client was 17 at the conclusion of treatment.

Expired Client Consent & Consultation Forms are destroyed by use of a shredder.

Use of cookies and other technologies

This website is hosted on the Wix platform. Wix is the online platform that allows William to communicate with you. Your data may be stored through Wix’s data storage, databases and general applications. Data is handled on secure servers behind a firewall. Web site usage and performance is monitored using Google Analytics.

 

A cookie consent banner is presented to the website user when first accessing the site and the user has the option of providing consent to use of cookies. Cookie permissions can be subsequently adjusted by the user within their browser settings.

Your rights in relation to personal data

Willpower Sports Rehabilitation & Conditioning reserves the right to modify this privacy policy at any time so please review it frequently. Changes and clarifications will take effect immediately upon posting on the website. If material changes are made to this policy, you will be notified that it has been updated so that you are aware of the information that is collected and how it is used.

The rights of the client or prospective client under data protection law are listed below. You are advised to read the relevant laws and guidance from the regulatory authorities for a full explanation of these rights.

Your principal rights under data protection law are:
